The community Neuro Service (CNS) provides rehabilitation and management for people who have a neurological condition including stroke in their own home or appropriate community location. The service provides specialist, coordinated advice and treatment to maximise independence, limit disease progression, reduce hospital admissions and facilitate smooth transition home following a hospital discharge.
Main Duties:
- To provide specialist speech and language therapy (SLT) service to clients with developmental or acquired disorders, including those with traumatic brain injury or progressive neurological illness, demonstrating a high level of competence in the management of acquired disorders of communication and swallowing; and an in-depth knowledge of the physical, psychological and emotional consequences of disability.
- To provide support to clients with long-term tracheostomy in the community.
- To manage and prioritise a complex clinical caseload independently.
- To develop and evaluate training for clients, carers, and team members on SLT interventions within the caregroup.
- To provide advice and support to junior SLT colleagues on SLT interventions for clients within the caregroup and their carers, including providing second opinions.
- To provide advice and support to clients, carers and the Multidisciplinary team on communication and swallowing issues
- To take a delegated lead and represent SLT in the development and implementation of service objectives and projects within the CNS including the development of Care Pathways.
- Take a lead role in the development of care protocols for generic rehabilitation workers.
- To develop the capacity and responsiveness of community based services for clients with disability in conjunction with the professional manager and service manager
